The Food Establishment Inspection Report is a weekly report that appears Sundays. Information is taken from reports provided by the Environmental Health Department, and individual reports can be viewed at its website, http://amarillo.gov/departments/community-services/environmental-health/foodinspections. A numerical grading system is now being used, with a score of 100 points being equivalent to zero demerits.

100 points – No major violation at the time of inspection

85 points – Considered an average score

70 points or below – Requires re-inspection within 24 hours

(A/98) Cenikor Foundation, 1001 Wallace Blvd. Food manager certification must be completed. Correct by 09/22.

(A/94) Chicken Express, 2106 2nd Ave., Canyon. Mold inside ice machine. Correct by 09/23. Back door must be self-closing and tight-fitting; multiple flies inside establishment; flies around and crawling on buckets of tea without lids; waste grease bin lids must be kept closed; leak at three-compartment sink must be repaired (repeat violation). Correct by 12/12.

(B/83) Denny’s #8557, 9601 E. I-40. Improper internal temperature of TCS (temperature/time controlled for safety) foods in front of make table; pancake mix and beef out of date. COS. Dried food on produce slicer; rust on metal ice scoop; small ice scoop at soda machine held in moldy bin; backflow device needs to be replaced on mop sink. Correct by 09/16. Personal cell phone on food prep surface; multiple food items without lids (repeat violation); items stacked wet; multiple surfaces in kitchen covered in old food debris (repeat violation); filters on vent hood need to be cleaned (repeat violation); thick grease dripping from vent hood; waste grease bin and area around it extremely dirty and lid open. Correct by 09/23.

(B/83) Dong Phuong, 2218 E. Amarillo Blvd. Unlabeled cart of food in back office out of temperature; roasted ducks and ready-to-eat foods at front door out of temperature. Correct by 09/12. Jars of tea on shelf out of date; no paper towels at back hand sink by mop sink; several pieces of tape on back vegetable coolers inside touching food; several items sitting on cardboard in walk-in refrigerators. Correct by 09/19. Gaps at back door and warehouse door must be sealed; frozen shrimp thawing improperly; scoop handles touching food product in dry spice containers; ceiling above dry storage still has visible damage (repeat violation); no signs at hand sinks indicating for hand washing only. Correct by 12/08.

(B/87) El Tropico, 1111 E. Amarillo Blvd., # C. Tripe in vinegar and pickles sitting out at room temperature; chlorine sanitizer not at proper strength. Correct by 09/16. Tripe in vinegar must have an approved HACCP (Hazard Analysis Critical Control Points) plan; paper towel dispenser not working; back door needs new weather strip; personal drinks and cell phones stored next to food on food prep surface; date stickers must be removed from dishes before washing; knives, forks and spoons not prewrapped must be presented so that only handles are touched by employees and consumers if consumer self-service is provided. Correct by 09/23.

(B/87) Garden Fresh Fruteria La Hacienda, 1821 S.E. 3rd Ave. Several packages of different beef cuts green and spoiled (repeat violation); bleach cleaner stored on food contact surface; no allergen labeling on peanut snacks (repeat violation). COS. Several household pesticides in establishment. Correct by 09/15. Several food items without labels containing common name of food. Correct by 09/22. Gaps at back receiving door must be sealed (repeat violation); tongs stored opened on dirty pipe next to back hand sink; back ceiling over three-compartment sink is not smooth, durable and easily cleanable; exposed wood shelf above three-compartment sink and prep table in back are not smooth, durable and easily cleanable. Correct by 12/11.

(A/95) Heights Discount & Café, 1621 N.W. 18th Ave. Raw meat stored improperly in coolers. Correct by 09/18. Sanitizer test strips needed. Correct by 09/25.

(A/97) Hud’s, 4411 Bell St. Employee drink without lid and straw; boxes in fridge and freezer stored on floor; scoop and plastic cup stored in jalapenos. Correct by 12/13.

(A/90) Kabuki Romanza Japanese Steakhouse, 8130 W. I-40. No soap at hand sink on cook line. COS. pH records for all batches of sushi rice must be kept up to date; cutting board in cook line or kitchen heavily scored and must be sanded or replaced. Correct by 09/22. Pests and bugs in light covers in vent hood; gaps at both back doors must be sealed; personal keys on food prep area; debris build-up on fire suppression system in vent hood; heavy build-up of ice on walk-in freezer condenser. Correct by 12/11.

(A/98) Kirkland Court Health & Rehabilitation Center, 1601 Kirkland Drive. Back door is catching on tile and not fully closing; need to clean inside ice machine. Correct by 12/11.

(B/85) Little Caesar’s Pizza, 1401 E. Amarillo Blvd, #A. No sanitizer available. COS. No hand washing observed for entire duration of inspection. Correct by 09/17. Food handler certification needed; door not latching on reach-in cooler and gasket in disrepair; lid to chest freezer coming apart and exposing fiberglass; no paper towels at hand sink; back door not self-closing and frame severely damaged; sauce container stored on floor inside cooler; mold in walk-in cooler near condenser unit. Correct by 12/13.

(A/99) Murphy Express #8685, 2109 S. Western St. No hand washing sign in employee restroom. Correct by 12/13.

(A/97) Opportunity School Central, 1100 S. Harrison St. Sanitizer bucket stored in hand sink. COS. No hot water at hand sink in kitchen area. Correct by 10/03. Acoustic ceiling tiles must be changed for vinyl coated tiles or other type of non absorbent material. Correct by 12/11.

(A/91) Pilot Travel Center #723, 9601 E. I-40. Heavy mold build-up on soda nozzles; hot dogs out of date; no paper towels at front hand sink. COS. Back double doors need new weather strip; need to clean microwaves, hot chocolate machines and vents; soda syrup under soda boxes. Correct by 12/12.

(A/91) Pizzeria Nomad, 2648 S.W. 34th Ave. Refrigerator below cash register holding at improper temperature. Correct by 09/17. Hard water stains and rust inside ice machine. Correct by 09/24. Employee drinks in walk-in fridge, freezer, refrigerator under cash register and near food storage in back; ice scoop in bin of room temperature water; old food spillage on stove (repeat violation); ceiling is peeling in first walk-in cooler (repeat violation). Correct by 12/13.

(A/95) Pizza Hut, 110 23rd St., Canyon. Sinks and drain boards of ware washing sinks and machines must be self-draining. Correct by 09/23. Splash guard needed at hand sink; extensive mold growth on make table cooler; heavy mildew build-up on dishwasher racks; mold on walls in ware washing area (repeat violation); vent hood filters must be cleaned (repeat violation); sewage clean out cap at back door is loose and needs to be tightened. Correct by 12/12.

(B/86) Pho 84, 5713 S.W. 34th Ave. Several opened sauces sitting on shelving not refrigerated (repeat violation). COS. Thick coating of burned food on grill (repeat violation); dishwasher leaking and dripping onto floor and machine is not correctly pulling sanitizer. Correct by 09/22. Exterior door is closed by a piece of wood and does not have a seal; shrimp thawing improperly; scoop handles touching food products (repeat violation); layers of dried product on soda and tea dispensers (repeat violation); visible splash marks and grease build-up on several sites on ceiling and walls (repeat violation); intake and exhaust air ducts must be cleaned and filters changed (repeat violation); trash piled next to grease interceptor; mop bucket stored upside down in sanitizer sink (repeat violation); tools stored next to food products for customers (repeat violation). Correct by 12/11.

(A/93) Sonic Drive In, 1009 E. Amarillo Blvd. Food contact surfaces must be clean to sight and touch. Correct by 09/17. Many items stored uncovered; surfaces of utensils and equipment contacting non-TCS food must be cleaned every 24 hours (repeat violation); nonfood-contact surfaces of equipment must be kept free of accumulation of dust, dirt, food residue and other debris (repeat violation); employee restrooms must be cleaned every 24 hours (repeat violation). Correct by 12/13.

(B/85) Suzy’s African International Market, 3318 S. Georgia St. Several dented cans and one with food coming from seal. COS. Ice crystals in fish (evidence of fish having been thawed and refrozen). Correct by 09/15. Fish not labeled with what type of product or where it was from (repeat violation); many items with expiration or best by dates several months or years past date; no hand drying provisions at two hand sinks; no soap at hand sink. Correct by 09/22. Mice feces near refrigerator in back; three dead mice near freezer in back; several freezers with condensation build-up; dripping stains on inside several refrigerators; blood on floor that had dripped from some food product. Correct by 12/11.

(B/86) Taco Bell #032693, 3309 S. Washington St. Mexican pizza sauce in freezer at improper temperature; mold on bottom, spout area of Icee machines. Correct by 09/15. Grease trap lid broken off and must be replaced. Correct by 09/22. Food floating in oil in fryer and oil had turned almost black (repeat violation); dishes stacked wet (repeat violation); rusting shelves next to hot hold need to be replaced (repeat violation); ventilation hood filter and drip pans above fryers are dirty and visibly dripping into fryers (repeat violation); freezer leaking onto ground. Correct by 12/11.

(A/92) Taqueria Los Toritos, 2617 E. Amarillo Blvd. Raw chicken stored above produce and other ready-to-eat items in reach-in. COS. No Certified Food Manager. Correct by 09/23. All food items must be covered when stored; scoop handle touching sugar; men’s restroom door must be self closing, Correct by 12/12.

(B/85) Thai Pepper, 311 E. Hastings Ave. Food in make table at improper temperature; trays used for holding chicken were being reused to drain grease off food; no sanitizer available. Correct by 09/16. Several roaches in establishment; oil and garlic sitting at room temperature (a HACCP plan is needed); cabinet holding hand sink has wood that is chipped, pitted, pilling, has black mold and a hole. Correct by 09/23. Cat food thrown outside building; skirt around bottom of building is not tightly sealed and has cats going in and out from under building; vegetables stored under raw and cooked meat product. Correct by 12/12.

(B/88) The Lost Cajun, 2401 W. I-40. Several chemical spray bottles without labels. Correct by 09/17. Ranch dressing and other products in walk-in cooler out of date; back hand sink blocked by trash can and towel bucket; thick layers of debris on grill and stove; mold inside ice machine. Correct by 09/24. Several scoops without handles or handles touching food. Correct by 12/13.

(A/96) Twins Discount 2, 1312 N. Hughes St. No paper towels or soap at hand sink (repeat violation). Correct by 09/24. Wood used to hold up water softener must be painted or stained so it is non-absorbent; bathroom doors must be self closing. Correct by 12/13.

(A/97) Wesley’s Bean Pot & BBQ, 6406 River Road. Employee food and drinks must be stored in a designated area; scoop handles in beans; ceiling tiles in bathroom must be smooth, non-absorbent and easily cleanable; wall behind sink must be refinished so bare wall is not exposed. Correct by 12/12.

(A/90) West Plains High School-Field House, 10576 Arden Road. No sanitizer for ware washing machine; several employees not washing hands. Correct by 09/17. No supervisory employee/volunteer with food handler certification was present during inspection. Correct by 09/24. Several flies in facility; food handlers not properly restraining hair.
